Help reporting an incident of neglect

So recently there an incident that occurred at my clinic involving neglect of a client (from a BT) that I feel my admin has not handled with enough care. Based on other actions from this BT I feel it is warranted for me to report this incident especially since no one else has. I’m not 100% sure of the process of reporting incidents of neglect/abuse though so if someone could let me know what information I would need prepared and who exactly to contact about this that would be greatly appreciated.
