Issues with Xbox One Save File reverting/not saving

For the 2nd time now I have lost several hours of progress on a character due to an issue where the game either reverts or just didn't save my data at all while I was playing. I'm not sure what causes this issue nor how to replicate it. I went from level 25 back to level 21. This is a warning to people to make sure their game is actually saving by quitting to menu every once in a while and checking they can load their character.

I've seen that cloud saving is an issue on PC, but I wonder if anybody else has had a similar experience on console, and if there is actually a way to know if your game is saving while you're playing.
